 Please hand in the completed application form to the school’s General Office, together with the following documents between 3 January (Monday) and 17 January (Monday) 2022 before 4:30p.m.:

1. 1. the application form for S1 Discretionary Places (provided by EDB) with all three parts intact;
   2. the original of the identity card / birth certificate of the applicant for checking by the school; (Photocopy only if application is made by mail)
   3. photocopies of report cards of primary 5 (both 1^st & 2^nd term) and primary 6 (1^st term);
   4. photocopies of awards, certificates or records of extra-curricular activities (if any, ten items at most, please refer to part II);
   5. Recommendation letter from sponsoring body (if any); and
   6. two self-addressed stamped envelopes (size 9”×4’’ with $2 stamp).
   7. (3 self-addressed stamped envelopes if application is made by mail)

In view of the COVID-19 epidemic situation, parents can submit the completed Application Form and the above documents to Ho Lap College (Sponsored by Sik Sik Yuen) by post between 3 January (Monday) and 17 January 2022 (Monday), both dates inclusive (the postmark date should not be later than the prescribed deadline). Please indicate on the envelope:

Parents of successful applicants will be notified of their children’s inclusion in the Successful List for Discretionary Places by letter and phone on 31 March 2022 (Thursday).
